I have started as an inpatient coordinator in Optum where we check the facilities where the member stayed during their hospitalization. Gathering information through different department within the facility for accurate documentation of their stay until they leave the facility. Following my next experience with another company where we call the members to offer assistance regarding their medication refill. We also send fax for provider's follow up for a new prescription. We call their pharmacy with their discretion and offer consult to our pharmacist department when member mentioned they are experiencing side effect from the maintenance medication that they are taking.
