Ensures HHA notes are submitted timely & in accordance with company policy.
Requests & reviews patient clinical records & documents to ensure proper care planning.
Assembles patient Plans of Care (485) in accordance with company policy.
Ensures patient visit frequencies are correct & tracks missed visits.
Coordinates activities related to clinical record review for data collection.
Audit charts for clinical oversight, compliance and adherence to the US Medicare guidelines.
Identify clinical problem areas and document deficiency trends.
Communicate with clients’ administrative and clinical associate staff in the correction of any such deficiencies.
