Rate: $4/hr, and earn 40,000 PHP +/month.

Responsibilities:
- Conduct telephonic follow-up with patients and/or caregivers, identifying clinical and social needs and/or gaps in care
- Partner with social and clinical colleagues, PCP, and hospital-based clinical teams to help coordinate internal and external resources needed to manage the patient’s plan of care
- Perform triage assessments, escalating acute issues to Advance Care Provider team
- Assist with scheduling PCP/specialist follow-up
- Respond to patient and/or caregiver phone calls
- Provide education to patients and caregivers
- Conduct clinical intake visits with prospective patients via video and/or phone. Intakes include verifying and documenting a clinical history, verifying medications, answering basic questions about services, and identifying and flagging concerning history or labs for provider review.
- Educate patients about their journey with us, setting them up for clinical success.
- Monitoring and assigning provider support requests and initiatives.
- Providing updates to clinical intake workflow
- Delivery of just in time feedback as needed

Good to Have Qualifications:
- California-licensed Registered Nurses
- Telehealth or phone triage experience is a HUGE PLUS (but not required)
- Experience with CareXM
