PH RN - Bilingual Spanish Language

We are looking for a Spanish Bilingual Registered Nurse to work as a contractor on a Chronic Care Management team. This person will interface with US physicians, physician’s US staff, and Virtual Ventures Support, Ph staff in providing chronic care to a group of patients. You may also be providing Remote Patient Monitoring services involving the use of patient home monitoring equipment. You will help in the support and administrative activity to ensure data is properly collected, communicate with patients to discuss the monitoring data and help interpret and explain to the patient. You’ll need to be adept at accessing internet based applications to obtain and review data. You will be given training and support in any systems you need to work with to deliver the services as required. You will be expected to exceed KPIs in delivering quality and timely care to patients that are observed outside of their standard readings.

Responsibilities:

? Work with physician to implement standards of care within a CCM regimen

? Support state level regulatory clinical standards for RPM care

? Work with RPM team members to provide full Medicare RPM patient support

? Record all activities within the RPM and EMR systems for each patient

? Provide documentation and supporting documentation required for Medicare and Insurance reimbursement

Qualities:

? Able to work independently and virtually from home

? Can understand and speak Spanish.

? Capable of mastering technical tools and applications needed to work remotely
