Virtual Healthcare Community Outreach Specialist
Job Summary
The Virtual Healthcare Community Outreach Specialist is responsible for fostering relationships, promoting health and wellness initiatives, and building partnerships with healthcare facilities, including home health agencies, hospice providers, and other community organizations. This role is conducted remotely, leveraging digital tools to engage with diverse populations, host virtual events, and connect individuals to essential healthcare resources.
Key Responsibilities
• Build and maintain strong relationships with healthcare organizations, community leaders, and local facilities through virtual communication channels.
• Act as a virtual liaison between the healthcare organization and partnering facilities to facilitate access to care and resources.
• Proactively reach out to facilities such as home health agencies, hospice providers, skilled nursing facilities, and community organizations to establish partnerships.
• Collaborate with facility representatives to identify mutual goals and develop cooperative initiatives to benefit the community.
• Schedule and conduct virtual meetings, presentations, and follow-ups with potential partners.
• Plan, coordinate, and host webinars, virtual health fairs, and online workshops for community members and partner organizations.
• Deliver tailored presentations about available healthcare services and programs.
• Assist individuals and families in navigating healthcare systems virtually, providing referrals and resources as needed.
• Ensure seamless collaboration between partnering facilities and the healthcare organization for enhanced service delivery.
• Work with stakeholders to assess community health needs and design virtual outreach initiatives to address gaps in service.

Qualifications
Education: Bachelor's degree in Public Health, Social Work, Communications, or a related field (preferred).
Experience:
• 2+ years of experience in community outreach, partnership development, or public health.
• Experience working in a virtual environment is highly desirable.
Skills:
• Strong interpersonal and communication skills, with the ability to build and sustain professional relationships remotely.
• Proficiency in virtual tools such as Zoom, Microsoft Teams, Google Workspace, and social media platforms.
• Effective organizational and project management abilities to coordinate outreach and partnership initiatives.
Technical: Familiarity with CRM software and virtual collaboration tools.
