The RN Clinical Record Coordinator is responsible for maintaining, organizing, and ensuring the accuracy and confidentiality of patient clinical records. This role involves managing electronic and paper records, ensuring compliance with legal and regulatory requirements, and facilitating communication between healthcare providers, patients, and administrative staff.
Key Responsibilities:
• Clinical Record Management:
o Ensure all patient records are accurately maintained in compliance with HIPAA regulations and Alvita Care policies.
o Ensure proper documentation of Initial assessments, physician orders, care plans, and patient progress notes.
o Review and Audit clinical records for accuracy, completeness, and compliance with agency policies.
o Ensure timely submission of all form for Initial assessments, physician orders, Plan of Crae updates, and Skilled nursing note, nurses notes, ADL forms.
• Regulatory Compliance & Quality Assurance:
o Ensure records meet state, and accreditation standards (e.g., DOH, Joint Commission).
o Identify and address documentation deficiencies and provide corrective action as needed
o Ensure Proper follow-up on documentation discrepancies and missing documentation/form
o Participate in quality assurance activities to improve documentation accuracy.
o Implement process improvement for enhanced record-keeping efficiency
o Assist in policy updates and training of nurses and HHAs regarding documentation protocols
o Assist in preparing for audits and surveys by regulatory agencies.
• Communication & Coordination:
o Work with nurses and home health aides to ensure proper documentation.
o Serve as a liaison between the home care agency, healthcare providers, and insurance companies regarding medical records.
o Respond to requests for medical records from authorized personnel and agencies.
• Administrative Duties:
o Maintain confidentiality and security of all patient records.
o Process medical record requests, releases, and corrections per agency policy.
o Assist in training staff on proper documentation procedures for Initial Assessments, Plan of Care updates, nurses' notes, ADL forms
o Track and monitor document submissions to ensure timely updates.
o Fax physician orders to patient primary care physician for signature.
o Ensure faxed physician ordered are returned and signed by physician in a timely manner.
Qualifications & Skills:
• Education:
o RN License
o Associate’s or bachelor's degree in nursing
Experience:
o Previous experience in medical records management, preferably in a home health or healthcare setting, preferred
o Knowledge of medical terminology, HIPAA compliance.
o Prior experience with clinical record management, compliance auditing is a plus
• Skills:
o Strong attention to detail and organizational skills.
o Excellent communication and interpersonal skills.
o Ability to handle sensitive patient information with discretion.
o Proficiency in using electronic health record (EHR) software and Microsoft Office.
