We are looking for Community First Services and Supports (CFSS) Program Coordinators. This is a great position for a case manager seeking to empower people to use services in their own communities. A successful candidate will enjoy meeting new people; have excellent interpersonal skills; deliver detailed work, stay organized and meet deadlines; and have a strong desire to successfully deliver services within the required guidelines of state statutes.

- Develop and maintain strong relationships with clients, acting as their primary point of contact.

Education and Experience:
Four-year degree in human services or related field with one year for direct care to individuals with disabilities or individuals of 65 years of age or older
OR Two-year degree in human services or related field with two years of direct care to individuals with disabilities or individuals of 65 years of age or older.
Demonstrated interpersonal skills that quickly build rapport with customers, county social workers, and administrative staff.
Case management experience related to self-directed services preferred.
Candidate must complete and pass a background study, have a valid driver’s license, proof of motor vehicle insurance and reliable transportation for work purposes.

8 hour shift
