Home Health Case Manager

FeldCare is a home therapy provider network located in California. We are looking for a full time, 40+ hours a week, Care Coordinator with Home Health experience. A Care Coordinator is similar to a case manager in other healthcare Upgrade to see actual infore Coordinators are responsible for overseeing active patient-care in order to identify and resolve issues that may arise during the patient’s care. They then communicate and coordinate solutions to these problems with the other parities responsible and involved in the patient’s care. As mentioned, Care Coordinators must have a strong clinical understanding, as well as critical thinking abilities, time management, and operate well under pressure.
